Tigertriever in Large Vessel Occlusion Treatment: A Systematic Review and Meta-analysis of Efficacy and Safety

Introduction: Acute ischemic-stroke due to large vessel occlusion (LVO) is an important medical emergency. Mechanical thrombectomy plays a crucial role in achieving revascularization in this condition and endovascular therapy has become the gold-standard in its management. The aim of this study is to evaluate the efficacy and safety of Tigertriever (Rapid Medical, Yoqneam, Israel) for treatment of LVO.

Methods: A comprehensive search following PRISMA-guidelines was conducted across Medline, Embase, and Web of Science databases for studies reporting on the use of Tigertriever for LVO treatment. Pooled-analysis with 95% confidence intervals (CI) was used to assess the effects. Heterogeneity was evaluated using I² statistics and a random-effects model was used. Successful reperfusion, technical success, clinical outcomes, complications, and mortality were assessed. Good clinical outcomes were defined as patients with a modified Rankin Scale ≤2. Complications were divided into major and minor intraprocedural, total-major, total-minor, and total-complications.

Results: Seven studies, comprising 315 patients, were included. The overall-rate of successful reperfusion was 86% (95% CI: 79% to 94%, I² = 80%), and of technical success was 83% (95% CI: 79% to 88%, I² = 0%). For good clinical outcomes 48% (95% CI: 32% to 64%, I² = 90%). For total-complications 21% (95% CI: 12% to 30%, I² = 81%). For major and minor intraprocedural complications 0% (95% CI: 0% to 1%, I² = 0%) and 3% (95% CI: 0% to 8%, I² = 70%), respectively. For total-major and total-minor complications analysis revealed a rate of 9% (95% CI: 2% to 16%, I² = 80%) and 11% (95% CI: 3% to 19%, I² = 87%), respectively. For mortality 17% (95% CI: 13% to 21%, I² = 0%).

Conclusion : The findings of this study provide robust evidence of high rates of successful reperfusion and technical success with low major complications and mortality rates when using Tigertriever for LVO treatment.
